Lewis County Public Health is conducting a 2024 Family Health Survey to understand needs and priorities for families in Lewis County.
The survey is available in Spanish and English. It can be completed online or on paper from now until June 1, 2024. Visit lewiscountywa.gov/familysurvey to complete the online survey or download the paper version.
